Please note that York Region Public Health is no longer sending vaccination consent forms to schools, rather they have been mailed directly to families with children enrolled in Grade 7 and Grade 8. Similarly, the consent forms are no longer returned to schools, rather they are completed and returned directly to York Region Public Health. If your child enrolled or transferred to the school late and has not received these forms by mail, please contact the York Region Public Health at 1-877-464-9675 ext. 73452.
